The Middlesex London Primary Care Network (MLPCN) is excited to bring together physicians, nurse practitioners, and health-care partners for a day of engaging and empowering conversation around Chronic Disease Prevention and Management (CPDM).
The 2025 MLPCN Primary Care Symposium – scheduled for Wednesday, October 1st (8 a.m.- 4 p.m.) at Dorchester’s Quiet Earth Farms – will feature practical, evidence-based education focused on modifiable risk factors, prevention strategies, and tools for supporting self-management in chronic disease care.
Key highlights of the day will include:
ü HealthPathways – Take a test drive to learn how these new resources support clinical decision-making and enhance patient care. If you have any technical questions, want to speak to a clinical editor, or wish to sign up we can walk you through all of that.
ü Community Program Connections – Discover how to integrate and align with local programs to increase capacity and improve outcomes in primary care.
ü Networking and Collaboration Opportunities – Build stronger ties across the primary care landscape.
ü Keynote Speaker: Dr. Amanda Marrone - Family physician and board-certified Lifestyle Medicine physician, Dr. Marrone will share powerful insights and practical advice from her experience integrating chronic disease prevention into everyday family practice.

Bridging Family Medicine and Speciality Practice for Efficiency and Enhanced Outcomes
Dear Colleagues,
Please join your District 2 Executive as we present a unique strategic session of how Family Physicians and Specialists can work together to optimize clear communication, ensure proper documentation, streamline processes, and reduce admin burden for better patient outcomes, while boosting the bottom line.
The session will include an overview of how the Pediatric Otolaryngology team has been utilizing a centralized referral for nearly 5 years in addition to sharing OR cases to help reduce wait times and how this model can be potentially transferred to other specialties.
OMA President, Dr. Zainab Abdurrahman, will also provide an OMA update with Q and A afterward.
